Family Support Services

Family Support Services focus on keeping families together and helping individuals live at home in the community. Services include information and referral, assistance with needs identification, advocacy, and support in accessing services like respite care, social activities, and specialized equipment. 

How it Works:

• Individualized Plans: Family Support Coordinators work with families to develop personalized plans tailored to their specific needs.
• Community-Based: The focus is on maintaining families in their homes and integrating individuals into the community.
• Variety of Supports: Services can include assistance with respite care, social and recreational activities, family education, and obtaining specialized equipment or supplies.
• Financial Assistance: Financial support for various needs, including vocational training and assistive technology. 

Services May Include:

• Information and referrals
• Assistance in identifying needs
• Advocacy to secure necessary services or benefits
• Help in locating and acquiring services such as:
  • Respite Care – In-home/Out-of-home
  • Social/Recreational Activities
  • Summer Camp
  • Counseling – Family/Individual
  • Family Education
  • Specialized medical equipment, medical supplies, clothing, nutrition, and personal care items
  • Diagnostic services
  • Therapeutic Services – OP/PT
  • Behavior consultation and support services
  • Dental services
  • Transportation and travel reimbursement
  • Environmental modifications – vehicle or home
  • Other services and supports unique to each family’s needs
